Most likely, the Inspector overlooked that it was a self-portrait.
We suggest making it very obvious that it is a self-portrait release. No witness is required for self-portrait releases so it may actually help if you prepare a release that has NO witness signature on it, because in the process of inspecting the release, it becomes a habit for Inspectors to automatically compare those dates. Instead, leave the witness lines blank and mark prominently in large letters across the bottom of the release, in red ink, "Self-portrait - no witness required." This extra visual clue helps a LOT when we're inspecting release after release and will help you avoid Inspector errors like this.
